My doctor has ordered a sleep study for me, what do I need to do?

(1) First we need your insurance information - usually your physicians office sends this information to us automatically, but we may on occasion need to get more information from you. You can fill out this form on-line and we can proceed with your insurance verification.

(2) Next, we need you to fill out a questionnaire that will be evaluated by our Medical Director to see what type of study we need to perform. Based on your answers you may be requested to stay during the day for a series of tests to evaluate your daytime sleepiness.

Is this covered by my insurance? We will check your eligibility with your insurance company prior to scheduling your study. We will not perform any studies unless they are covered. You will not be liable for any expenses that your insurance company does not approve.
